Jack E. Weaver, M.D.

Primary Care – Sports Medicine

A native of Muskogee, Dr. Weaver is a 1988 graduate of Oklahoma State University and a 1993 graduate of the University of Oklahoma College of Medicine. After completing a residency in family medicine at Michigan State University, Dr. Weaver completed a fellowship in Sports Medicine in June 1997 at the University of Oklahoma College of Medicine – Tulsa through Eastern Oklahoma Orthopaedic Center. He is a Diplomat of the American Board of Family Practice and has a Certificate of Added Qualifications in Sports Medicine. He is a member of the American College of Sports Medicine and the American Medical Society for Sports Medicine.
